This PDF has two pages. The first one is meant for younger kids and is partially filled out with ideas for them to choose from.
 
The second page is meant for older kids and is mostly blank so you can either fill in siggestions you are comfortable with or have your child/teen fill in some of their favorites.

Instead of printing  off each week, I encourage you to laminate the final version and choose each week with a dry erase marker!
